In a new video series, Mike Stahl will be discussing this year’s strategic plan: where we’re at and where we’re going. In the videos, Mike will give a short recap and below you be able to read more in-depth details. While this may be lengthy, the hope is that you can dig deeper into the issues that matter most to you and that everyone has a better understanding of Hilltop’s Strategic Vision. This week’s installment is about Our People:
Senior Life Options: Maintain/Increase Census, Pricing Models, Expansion
A primary focus is maintaining/increasing census in light of new local competition. The Fountains has experienced 17 move-in and 15 move-outs this year for a net gain of 2. The Commons has remained even with 24 move-ins and move-outs. In addition, Senior Daybreak’s census is nearly maxed out on a daily basis.
The new “entry fee” model was introduced and well received. Marketing plans will begin targeting independent living in The Commons in an effort to increase the occupancy of the studio units.
Expansion has continued with six patio homes on Fountain Hills recently completed in March. Three of the six already occupied. Plans are in the works to continue construction with the next eight patio homes later this fiscal year. Senior Daybreak in Montrose opened in a newly remodeled facility in November, and received state licensing and Medicaid approval.
Bacon Community Campus: Licensing & Credentialing, Campus Upgrades/ Life Safety, Campus Design
The State has changed direction for LAP licensing, moving from Assisted Living to Home Care regulations, thus prolonging the finalization of our license. We have toured the Director of Health Care Policy and Financing through the program, and have gained her full support for the LAP program, so we feel good about the possibilities. RYS is scheduled to have a CARF review and credential the program between March and May. This will hopefully push the State to increase the daily DHS rates.
Upgrades on the campus have continued throughout the year. The fitness center has been significantly upgraded with movie theater, game room, opened up common space and newly improved multiple purpose room. In addition, plans are nearly finalized to remodel the medication room and low stimulation dining room. A full campus fire suppression system is currently being installed with the support of a $1m grant from the Department of Local Affairs.
A master plan for the Bacon campus including the four adjacent lots is currently with the architect. This plan will need refining, cost estimates, and a timeframe to move this project forward in the next 18 months.
Family Resources Centers: Purpose, Community Integration, Expand Services
The Family Resource Centers continue to evolve through supervisory designs, new programs and utilization of space. A new organization chart was created after the loss of a couple long-term employees, streamlining the design. The FRC is eliminating program silos and focusing on the services provided.
Multiple new services are in the works. Pro Bono has been fully integrated into the FRC and 211 moved in last month. In addition, Multi Systemic Therapy (MST) and a licensed community day treatment program will begin seeing clients in the near future in the newly remodeled “west wing”. Other programs have been discussed with community stakeholders to move in once the timing is appropriate.
Significant effort and headway is being made working with Rocky Mountain’s Region Extension Area (REA). This has opened up opportunities for funding in new areas as well as some of our existing services. Billing rates and codes are just now being finalized for use in the near future.
Community Leadership: Support Key Initiatives, Build Workforce, Maintain High Organizational Reputation
Hilltop leadership participates on countless boards (state/local), advisory committees and community efforts. Hilltop is often asked to be “at the table” for brainstorming, assessing or implementing community efforts.
Currently, leadership is developing a tool to measure Hilltop’s reputation and leadership within the community. It is anticipated to have this tool completed by summer.
