How do I get another medical insurance card?

You can go onto UMR’s portal yourself and print another medical card.  www.umr.com OR Call Susan, Hilltop’s Benefits Coordinator – she can print you a temporary card and order a new one to be sent to your home. 

 

How do I find a Doctor or a Specialist?

Visit the UMR website designed specifically for Hilltop employees and plans: CLICK HERE

OR you can reach out to Monument Health who can help you choose a Primary Care Physician and discuss Specialists in our plan. Their phone number is 970-683-5630. 

 

Can I use Telemedicine?

Yes. If you have Hilltop’s medical insurance, you can use Teladoc with $0 copay.  Refer to page 9 in the Benefit Guide for instructions on how to sign up for Teledoc. You can also use Telemedicine to meet with your Primary Care Physician remotely.  Each office handles this differently, but you pay your normal co-pay, and use technology to receive care.

 

I need a preauthorization for a procedure, what do I do?

Your doctor will need to call UMR to authorize this procedure. Provide your doctor your insurance information.  

 

What is the difference between Tier 1 and Tier 2?

These are the two networks that we offer our employees, tier 1 offers the best discounts and lowest deductible and out of pocket for medical use. Tier 1 doctors are normally affiliated with St. Mary’s where tier 2 are normally affiliated with Community Hospital.

 

Who is an “eligible dependent”?

  • Your spouse
  • Your children under the age of 26
  • Your “Common Law” spouse / “Domestic Partner” (Additional paperwork needs to be completed

 

What do these words mean?

  • Specialist – this is a provider that specializes in a specific area of medicine. We no longer require referrals to see specialists
  • Deductible – this is the amount of money you will pay before your insurance starts paying. You will then pay a portion of the costs until you meet your out of pocket maximum. 
  • Out of Pocket Maximum – this is the maximum amount of money you will pay in the benefit year. After this is met, your insurance will pay 100% of the costs. 
  • Copay – this is the amount of money that you will pay for a service.

You do not need an insurance card for dental insurance. You will need to notify your provider you have Delta Dental Insurance and provide them your personal information. They will be able to find you in the Delta Dental system.

What is the difference between a 401K Traditional and a Roth 401K? Who can I reach out to, locally, that can help me with my 401k plan?

  • These are the 2 Retirement Plans that are offered to all Hilltop employees. Reminder: you can sign up anytime during your employment with Hilltop. (Not only during your 1st 30 days)
  • Traditional 401K –Pre-tax dollars allow assets to grow tax-deferred which means you won’t be required to pay taxes until withdrawals are made from the plan.
  • Roth 401K – Contributions are made after-tax. All future growth and qualified withdrawals are tax free.
  • Contributions can be made to both Traditional and Roth 401K at the same time.

What is PTO?

Hilltop combines all paid time off into one bucket. This bucket of accrued time can be used for any time away from work such as Holidays, Sick days, personal days, vacation days, etc. You can find your PTO accrual on page 23 of the benefit booklet.

Can I cash out my PTO bank?

Yes, you can cash out up to 40 hours of PTO per calendar year. 100 hours must remain in your bank after cash out.

 

What is a sabbatical?
